چکیده

Methods 61 patients undergoing valve surgery were managed with V-atriotomy incision because of inadequate exposure of the mitral valve and subvalvular apparatus. 37 patients were male (60.7%) and 24 patients were female (39,3%) with a mean age of 64.7±10,2. Standart left atriotomy incision was made primarily and in cases of bad exposure, caval snares were tightened for total perfusion. Right atriotomy incision was done as well followed by interatrial septum incision. Interatrial septostomy incision and left atriotomy incision were connected at 2 cm medial to inferior vena cava cannula. Suspensory sutures were placed to the free edges of right atriotomy and interatrial septostomy. This excellent visualisation of the mitral valve leaded us to perform valve repair or replacement procedures.

Advantage of a More Central Incision onto Left Atrium by Using Ultrasonic Scalpel

Exposure of the mitral valve is of critical importance in mitral valve surgery. The mitral valve is located near the interatrial groove. As we accumulated experience and learned from embryology, we found that a more central left atriotomy was advantageous, and was made possible by interatrial groove dissection using an ultrasonic scalpel. This crucial finding prompted us to report the technique.

Comparison of superior septal approach with left atriotomy in mitral valve surgery

OBJECTIVE In this study, we aimed to compare clinical outcomes of superior transseptal approach with the conventional left atriotomy in patients undergoing mitral valve surgery. METHODS Between January 2010 and November 2012, a total of 91 consecutive adult patients (39 males, 52 females; mean age: 54.0±15.4 years; range, 16 to 82 years) who underwent mitral valve surgery in the Division of C...

Repair of injured right inferior pulmonary vein during mitral valve replacement

During mitral valve surgery right pulmonary veins injury, subsequent to excessive traction (for better exposure of the mitral apparatus), is often unavoidable. This is more likely in patients with small left atrium. This common complication may cause severe intraoperative bleeding, while its surgical repair may lead to complications such as late stenosis or obstruction of the pulmonary veins. T...

The trans-septal approach to the mitral valve.

To assess the efficacy of the trans-septal approach to the mitral valve, 40 patients who underwent mitral valve surgery via this approach were compared to 37 patients who underwent surgery via the standard left atriotomy. Concomitant procedures included tricuspid annuloplasty, aortic valve replacement, closure of atrial septal defect, coronary artery bypass grafting and aortic valve repair. The...
